RADIESSE Patient Access Program
BioForm Medical, Inc. is committed to helping people with HIV-associated Facial Lipoatrophy receive RADIESSE treatment. To help patients afford treatment, BioForm offers a RADIESSE Patient Access Program to help patients afford Radiesse. See below for information on eligibility criteria and how to apply.
Who is Eligible?
In order to qualify for a discounted RADIESSE price under the Patient Access Program, a patient must:
- Have Facial Lipoatrophy associated with HIV-treatment
- Lack private or public prescription healthcare insurance coverage for RADIESSE dermal filler, such as HMO or PPO coverage, and
- Earn less than $80,000 per year
- Be a U. S. Citizen
- Be at least 18 years old
How Do I Apply?
- Visit a licensed HIV care physician in the United States who has completed RADIESSE training.
- Ask your HIV physician to complete the RADIESSE Patient Access Program Application form and attach your most recent proof of annual income, such as a Federal Tax Return.
- If approved, BioForm Medical will ship up to six 1.3cc syringes of RADIESSE filler at discounted pricing to your HIV care physician within two weeks.
Please Note : The cost of RADIESSE filler under the Patient Access Program depends on a patient's income. While BioForm Medical will make every effort to provide access to the RADIESSE Patient Access Program for eligible patients, the program is limited to available resources and may be discontinued or revised at any time without further notice.
